Module 3 will educate participants on the critical aspects of patient care documentation, emphasizing the legal and ethical significance. It covers the essentials of accurately and responsibly recording patient interactions, treatment plans, and progress notes, highlighting the necessity of using specific, measurable goals and objectives to track patient progress. Additionally, the module addresses the common pitfalls associated with employing non-standardized language or assumptions rather than behavior-based observations. Furthermore, participants will learn about the ASAM (American Society of Addiction Medicine) criteria, understanding its role in documenting and monitoring patient progress through the continuum of care.

The educational goal of Module 3 is to increase knowledge about proper patient care documentation and ASAM criteria.
At the end of this course, participants will be able to:

Describe the basic elements of and responsibilities regarding documentation of patient care in the medical record, recognizing it as a legal document.

Explain how documentation is used as a form of advocacy.

Explain 2 or more reasons why it is best practice to use specific, measurable goals/objectives, accompanied by behavioral descriptions of patient progress.

Describe 2 or more pitfalls of using non-standardized language (jargon/slang) and/or language based on assumptions rather than behavior.

Identify and describe the ASAM criteria and its use in documenting and tracking patients progress through the continuum of care.
