Description

The case management skill set is not taught in graduate or nursing schools. However, once clinicians are working in direct care, they realize that it is an integral component of their work. The compartmentalization of the behavioral health system is a complicating factor. Matching clients’ insurance/financial, clinical picture and motivation to the appropriate quality of traditional and non-traditional levels of care is crucial. This presentation is intended to organize this process and provide templates and a roadmap for all clinicians who are inevitably involved in care coordination and aftercare planning.
